Applying Performance Improvement Tools

Imagine that, for about a year, your nursing unit has been involved in an intensive campaign to improve patient satisfaction scores with pain management. You are getting good data from your patients, as the length of stay on this inpatient geriatric medical nursing unit is only about 6 days. Your hospital does 100% survey to inpatients, and the response rate is about 25%, which is higher than it has been. This notwithstanding, the percent of “patient very satisfied” (top box), with a score of 5, has been in the low 70s. The national benchmark for medical surgical units like yours is about 85% very satisfied. Of all the units in your hospital, your unit is the lowest scoring on this HCAPHS survey. But as your unit is the only geriatric medical nursing unit in the hospital, you’d always thought it was the nature of the patient population.
You have been the day shift representative to the QI team, and the scores on your unit are posted monthly. Here are the numerous strategies that have been tried on your unit and the timeframes.

Document: Strategies and Interventions (PDF)–attached below

For this Discussion, examine the strategies and interventions tried in your unit and consider the following questions: a) Were the strategies effective in creating a sustainable change on your nursing unit, and b) To what extent can your nurse manager and CNO count on your unit exceeding the national benchmark in the next quarter, the next year? That is, does this run chart have some predictive ability? Does the run chart support the nursing unit’s decision to celebrate? To what extent can the leadership be confident that the trend will continue?

By Day 3

Based on the scenario, explain what was done successfully and where improvement was needed in the quality improvement process. Identify the performance improvement tools, and explain how they contributed to the outcome.

Support your response with references from the Resources and professional nursing literature. Your posts need to be written at the capstone level (see checklist).

Advanced Pathophysiology Assignment

Week 7: Concepts of Neurological and Musculoskeletal Disorders – Part 1

Anatomists often use the analogy of a house to explain the human body, with skeletal systems, respiratory systems, and circulatory systems represented as a home’s framing structure, ventilation, and piping, respectively. Such analogies further emphasize the point that relationships between systems can result in complications when issues arise in one system.

With hundreds of diseases that can impact the brain, spine, and nerves, neurological disorders represent a complicated array of issues that present significant health concerns. Disorders such as strokes and Parkinson’s disease not only affect the nervous system, however; they can have secondary impacts in other areas, especially the musculoskeletal system.

This week, you examine fundamental concepts of neurological disorders. You explore common disorders that impact these systems and you apply the key terms and concepts that help communicate the pathophysiological nature of these issues to patients.

Learning Objectives

Students will:

· Analyze concepts and principles of pathophysiology across the li

Week 8: Concepts of Neurological and Musculoskeletal Disorders – Part 2

As homeowners know all too well, there is a continuous need for maintenance and repair. Some efforts are precautionary in nature, while others are the result of issues that surface over time.

Similarly, musculoskeletal disorders can develop over time. For some disorders, such as osteoporosis, precautionary treatments are a potential option. But much like issues that surface in a home over time, many musculoskeletal issues can be very serious concerns, and they can have a significant impact on patients’ lives.

This week, you continue to examine fundamental concepts of neurological and musculoskeletal disorders. You explore common disorders that impact these systems and you apply the key terms and concepts that help communicate the pathophysiological nature of these issues to patients.

Learning Objectives

Students will:

· Analyze processes related to neurological and musculoskeletal disorders

· Identify racial/ethnic variables that may impact physiological functioning

· Evaluate the impact of patient characteristics on disorders and altered physiology

Module 5 Assignment: Case Study Analysis

An understanding of the neurological and musculoskeletal systems is a critically important component of disease and disorder diagnosis and treatment. This importance is magnified by the impact that that these two systems can have on each other. A variety of factors and circumstances affecting the emergence and severity of issues in one system can also have a role in the performance of the other.

Effective analysis often requires an understanding that goes beyond these systems and their mutual impact. For example, patient characteristics such as, racial and ethnic variables can play a role.

An understanding of the symptoms of alterations in neurological and musculoskeletal systems is a critical step in diagnosis and treatment. For APRNs this understanding can also help educate patients and guide them through their treatment plans.

In this Assignment, you examine a case study and analyze the symptoms presented. You identify the elements that may be factors in the diagnosis, and you explain the implications to patient health.

To prepare:

Patient is a 24-year-old female administrative assistant who comes to the emergency department with a chief complaint of severe right-sided headache. She states that this is the sixth time in the last 2 months she has had this headache. She says the headaches last 2–3 days and have impacted her ability to concentrate at work. She complains of nausea and has vomited three times in the last 3 hours. She states, “the light hurts my eyes.” She rates her pain as a 10/10 at this time. Ibuprofen and acetaminophen ease her symptoms somewhat but do not totally relieve them. No other current complaints.

Assignment ( 2-pages case study analysis) 3 citations reqiured.

In your Case Study Analysis related to the scenario provided, explain the following:

· Both the neurological and musculoskeletal pathophysiologic processes that would account for the patient presenting these symptoms.

· Any racial/ethnic variables that may impact physiological functioning.

· How these processes interact to affect the patient.

Psychotherapy With group

As a future advanced practice nurse, it is important that you are able to connect your classroom experience to your practicum experience. This week, you begin documenting your practicum experiences in your Practicum Journal.

  • In preparation for this course’s practicum experience, address the following in your Practicum Journal:
  • Select one nursing theory and one counseling theory to best guide your practice in psychotherapy.
Note: For guidance on nursing and counseling theories, refer to this week’s Learning Resources.
  • Explain why you selected these theories. Support your approach with evidence-based literature.
  • Develop at least three goals and at least three objectives for the practicum experience in this course.
  • Create a timeline of practicum activities based on your practicum requirements.

Nutrition and drugs

Instructions

Consider the discussion questions below. Please begin a discussion about nutritional regulations and how they help the nutritional status of the population in the United States, (or don’t help it). Your initial comments must be at least a full solid paragraph for each question (over 50 words).

Here’s a good one to ponder while out and about?  Some cultures are very accepting of alcohol consumption while others find it taboo until you reach some age society has determined allows you to “control yourself.”  On the other side, we find the same strange condition concerning the operation of motorized vehicles.  Question:  Which would you rather have a restriction upon, alcohol consumption, or driving?  Please provide a rational reason for your choice.

Cultural Health Promotion Project

Cultural diversity refers to the differences among people based upon shared ideology and shared valued sets of beliefs, norms, customs, and meanings evidenced in a way of life. This diversity is expressed in many ways. Diversity in history, beliefs, practices, and opportunities not only exist among the many cultural groups, they typically exist across a wide continuum within the group. Health behaviors are influenced by culture and cultural values as well as socioeconomic status. In this assignment, students will develop a health promotion project for a cultural group living anywhere in the United States. You are to learn as much as you can about the culture norms and patterns and how these norms and patterns influence health practices. The purpose of this assignment is to develop a deeper understanding and sensitivity regarding the role of culture in patients’ lives and in nursing through the examination of a cultural/ethnic group while addressing health promotion strategies and disease prevention. PROJECT COMPONENTS The project should address the following Aspects of Cultural Assessment: 1. Ethnic/racial identity: How does the group identify itself in terms ethnicity and racial background? What is the range of interaction outside of the cultural group? Are recreational, educational, and other social activities within the ethnic reference group, the wider community, or both? 2. History and Value Orientation: How are values derived? What is the historical experience of the group that may have impacted values? 3. Language-communication process: What is the primary spoken language? What languages are spoken in the home? And by whom? What language is preferred when speaking to outsiders? Do second and third generations in the speak the language of their grandparents? 4. Health Beliefs and Practices: What are the traditional health beliefs? How common are these beliefs and practices within this group? To what extent are folk healing practices and practitioners used?  5. Religious Belief and Spirituality: Discuss religious articles & practices. How is spirituality is defined in the culture? Remember that Religious Beliefs and Spirituality are different concepts. 6. Lifecycle events: Which life cycle events are important to the culture? What are the customs associated with births, coming of age, marriage, and death? 7. Nutritional Behavior/Diet: Are there restrictions? Are there common practices? 8. Medical health care access and experience: Is access to care and equality of care an issue for this group? For example, are group members less likely to receive needed care due to system issues such as financial access or provider biases than another group? Based on the Cultural Assessment Data, the group will design a Cultural Health Promotion Plan: 9. Cultural Health Promotion Plan: Identify a health risk or health problem the project will address. Describe why this is an issue for your chosen target group and how cultural plays a role in the health risk or problem. 10.Health Promotion Strategies/Methods: Based on the health risk/problem identified, describe the strategies/methods that you will implement. Focus on primary and secondary prevention strategies. Refer to Healthy People 2020 Topics and Objectives. You may access the website at: https://www.healthypeople.gov. 11.Project Evaluation Methods: State the outcomes you expect to achieve. State how you will measure/assess the effectiveness of the project. This is a GROUP assignment. Groups will consist of no more than three students. A group leader must be assigned. Submit the paper via Moodle designated assignment link. Only one submission per group. DUE DATE: Week 12 Complete the paper according to APA 6th Edition Guidelines as a word-processed document. Assignment 08: Cross-Cultural Conundrum

HA3300D – Healthcare Policy and Ethics

Assignment 08: Cross-Cultural Conundrum

 Directions

You are the manager of your local regional health clinic. You have a meeting with your Board of Trustees coming up in a few days. Your trustees have asked you to prepare a short presentation in response to an incident that occurred at your clinic just a few weeks ago:

A young Native American woman fell sustaining an injuring to her arm. After a week, her arm is still painful which causes her to visit a physician at your clinic. The physician orders tests, ascertains that there is no fracture, and decides to simply advise over-the-counter medication for inflammation and pain. Two weeks later the patient returns with the same issue at which time the physician believes that physical therapy will help the injured arm. While undergoing physical therapy, the patient also visits with a local healer who prescribes herbal treatments for the injury. The patient returns to see the physician after two weeks with an improvement in her injury. On her last visit she wanted to discuss her desire to use natural treatments in conjunction with physical therapy with the white doctor but did not. In fact, she has discussed very little with her doctor about her injury, how it has affected her ability to care for her children and elderly mother, etc. She does not fully trust the white doctor and has heard him sound skeptical concerning healers from her culture or other types of “non-traditional” treatments. The patient does, however, feel that natural remedies administered by healers help her.

For this assignment, either prepare a speech by typing the transcript in a Word document, OR prepare a brief (7-8 slide) PowerPoint presentation. In your speech/presentation, address the following questions:

.What ethical issues does this incident raise?

.Should you be involved in this issue at all, or is this a job for another part of your organization?

.Propose a solution to the conflict, supporting your plan using ethical concepts.
