In the scenario, the nurse manager and the director of pharmacy blame each other for the error. The facilitator (quality assurance person) asks everyone to avoid blaming and focus on applying the tools to analyze the data and get to the root cause of the error. While all of these tools contribute, for this Discussion, select one tool to analyze.

By Day 3

Post each of the following:

  • Analyze the composition of the RCA team. Explain what knowledge they can contribute to the RCA.
  • Describe the collaboration in the case study that led to effective problem solving. Identify the evidence you observe in the scenario that demonstrates effective collaboration and the avoidance of blaming.
  • Explain the team’s process in testing for and eliminating root causes that were not contributing.
  • Select one of the performance improvement charts presented in the scenario and critique its effectiveness by explaining how it contributes to identifying the root cause and determining a solution to prevent repeat medication errors.
  • Identify the contributing factors, and discuss how to prevent this kind of error from occurring in the future.

Weeks 5 and 6 Electronic Health Records (EHR) Research Paper

 Review the history of electronic health records (EHRs) and discuss the pros and cons of EHR systems. How do EHRs help maintain accurate patient records? Which types are available to the health community? What are the ethical considerations needed when using EHRs? Support your responses with scholarly research. Use the Internet and the WCU online library databases to research peer-reviewed articles on EHRs. Compose a three- to four-page research paper in APA format. Your research paper should have a title page, an abstract, headings that delineate the specific assignment requirements, in-text citations, and a reference page. Cite a minimum of three references. 

 The paper demonstrates that the author fully understands electronic health records. The history of EHRs, including the different types of EHRs available, is discussed. Both the pros and cons of using EHRs is presented. Accuracy and ethical considerations with using EHRs are presented. All evidence is supported with timely scholarly research. 

 A clear and concise topic statement in the abstract provides direction for the paper Headings that delineate the specific assignment requirements are present. Concluding remarks demonstrate analysis and synthesis of ideas. Writing and integration of source materials is eloquent and skillful. 

Cost of Health Care

Our health, or the absence of such, can be very expensive. Many vulnerable populations simply do not have private funds available to pay for their care.  Relating your research to individuals with mental conditions, select a mental illness of your choice and discuss your response to the following questions:

  • What public financing resources are available to individuals with this particular mental condition?
  • What are the eligibility requirements and restrictions to receive these funds?
  • What happens if eligibility standards/requirements are not met by those with this illness?
  • Research and discuss two methods of alternative funding for care for this group. What are the benefits and limitations of the methods you discussed?

Your initial contribution should be 250 to 300 words in length. Your research and claims must be supported by your course text and at least one other scholarly source. Use proper APA formatting for in-text citations and references as outlined in the Ashford Writing Center.
