NUR 435Replies

REPLY 1( this first is about what I wrote in prompt week 6 nurs435)

Maria,

Nice work sharing how you would apply the collaboration model to this scenario. You incorporated the asking questions aspect well in your response. This week we read about the 3-2-1 approach that could be used in your scenario or even in personal conflicts as well. Share with us your thoughts on this approach and how it could help to resolve conflicts.

For this discussion, I chose to review the conflict scenario from Tina Chavez’s GSE DP. Specifically, I chose to focus on conflict scenario one. In this scenario, a group of four students are assigned a project that has to be completed by everyone. About one week into the project, the group is told that another student has been added to the group. The new student states that he can improve the project, but all original group members believe that their work is already good enough. Since everyone has to participate the new student is given a new task, but the new student does not want to do the task and leaves.

In this scenario, it would be appropriate to use conflict resolution model. I would use the Triangle of Satisfaction Model. The Triangle of Satisfaction Model is used to examine interests as they fundamental to the field of conflict resolution. When this conflict resolution model is used, it is used to examine the parties’s wants, needs, and concerns, or interests. It would be appropriate for each student’s interests to be examined. In this case, their common interest is to earn a good grade on the project. The new student wants to participate as well. This is one of the interests that he has, so common ground should be found to ensure everyone participates. If I were in the group I would invite the new student to participate and contribute ideas.

strengths and weakness

Assignment:

Exercises

  1. Discuss the resource-based view of competitive advantage. Why is it important to understand organizational differences to use this approach?
  2. Briefly define what is meant by competitive advantage. Are competitive advantage and sustained competitive advantage identical concepts? Compare and contrast the two concepts.

Professional Development

Case Study #12: AIDSCAP Nepal

  • Conduct an internal environmental analysis, identifying the value-creating strengths and weaknesses for each value chain component. Model your response after Exhibit 4-4 (p. 144) “Value Creating Strengths and Value Reducing Weaknesses.”
  • For each strength, assess the competitive relevance, using Exhibit 4-6 (p. 149) as an example.

Case Study gastrointestinal

Case Study #1

Gastrointestinal Disorders

T.H., a 57-year-old stockbroker, has come to the gastroenterologist for treatment of recurrent mild to severe cramping in his abdomen and blood-streaked stool. You are the registered nurse doing his initial workup. Your findings include a mildly obese man who demonstrates moderate guarding of his abdomen with both direct and rebound tenderness, especially in the left lower quadrant (LLQ). His vital signs are 168/98, 110, 24, 100.4° F (38° C); he is slightly diaphoretic. T.H. reports that he has periodic constipation. He has had previous episodes of abdominal cramping, but this time the pain is getting worse. Past medical history reveals that T.H. has a “sedentary job with lots of emotional moments,” he has smoked a pack of cigarettes a day for 30 years, and he had “two or three mixed drinks in the evening” until 2 months ago. He states, “I haven’t had anything to drink in 2 months.” He denies having regular exercise: “just no time.” His diet consists mostly of “white bread, meat, potatoes, and ice cream with fruit and nuts over it.” He denies having a history of cardiac or pulmonary problems and has no personal history of cancer, although his father and older brother died of colon cancer. He takes no medications and denies the use of any other drugs or herbal products.

. 1. Identify four general health risk problems that T.H. exhibits.

  1. Identify a key factor in his family history that might have profound implications for his health and present state of mind.

3.The physician ordered a KUB (x-ray study of the kidneys, ureters, and bladder), complete blood count (CBC), and complete metabolic profile. Based on x-ray and laboratory findings, physical examination findings, and history, the physician diagnoses T.H. as having acute diverticulitis and discusses an outpatient treatment plan with him. What is diverticulitis? What are the consequences of untreated diverticulitis?

4.While the patient is experiencing the severe crampy pain of acute diverticulitis, what interventions would you perform to help him feel more comfortable?

5. What is the rationale for ordering bed rest?

T.H. is being sent home with prescriptions for metronidazole (Flagyl) 500mg PO q6h, ciprofloxacin (Cipro) 500mg PO q12h, and dicyclomine (Bentyl) 20mg  4 tomes per day for 5 days.

6.For each medication, state the drug class and the purpose for T.H.

7.Given his history, what questions must you ask T.H. before he takes the initial dose of metronidazole? State your rationale

8.What is a disulfiram reaction?

9. What are the signs and symptoms of an allergic reaction

Informatics in healthcare

Considering your selected topic, what healthcare teams would be essential to help support your new project implementation? How would each identified team help support the integration and outcomes you plan to achieve? Consider aspects related to communication, collaboration and professional organizations for best practices in healthcare informatics.

Advanced Pharma Discussion 1 – 300 words

For this Discussion, you reflect on a case from your past clinical experiences and consider how a patient’s pharmacokinetic and pharmacodynamic processes may alter his or her response to a drug.

Post a description of the patient case from your experiences, observations, and/or clinical practice from the last 5 years. Then, describe factors that might have influenced pharmacokinetic and pharmacodynamic processes of the patient you identified. Finally, explain details of the personalized plan of care that you would develop based on influencing factors and patient history in your case. Be specific and provide examples.

Skill Building Activity: Ethics in Evaluation

Overview: All students will read the following HPE case study related to discerning between research and non-research during program implementation. There are many blurred lines between research and evaluation, so careful consideration of research and non-research activity is essential to ensure ethical practices are carried out at the onset of the program implementation and evaluation process.

Case Study: The CDC funded 5 health departments to develop programs to provide HIV prevention and referral services to HIV-infected persons. The project objectives included increasing the proportion of HIV-infected persons who know their HIV serostatus as early as possible after infection, providing HIV prevention services to infected persons to reduce the risk of transmission to others, and assisting HIV-infected persons in accessing medical and other needed services. Grantees were encouraged to tailor their interventions to local circumstances, and the funding supported local evaluations of the effectiveness of the interventions in each site.

The grantees provided an array of services to a mix of target populations based on their location. The CDC’s perspective was that the primary intent of the program was to provide services targeted to local community needs, and the primary intent of the evaluations was to improve those services. Thus, the project was deemed to represent non-research by the CDC. However, it was anticipated that, as a secondary benefit, the projects also would provide information—or lessons learned—that might be useful elsewhere.

Although the CDC did not consider the evaluation component of the program to constitute research, some participating health departments differed in this assessment and sought institutional review board (IRB) oversight. At sites where local IRB review was invoked, implementation was delayed up to 1 year. Confusion over whether the program constituted human subjects research resulted in the loss of formative pilot data at 1 site when a local IRB determined that appropriate approvals were not in place. Assignment: After reading the case study, write a discussion that addresses the following:

· What are the potential ethical considerations in this scenario? What are the main risks for participants in the program?

· Explain the unintended consequences that may occur from the participants’ perspective if the evaluation of the program is deemed research and informed consent is obtained? How might this change impact the participants’ behaviors, attitudes, and/or trust in the program?

· What are the unintended consequences of labeling this evaluation as non-research? How might this impact the program or future programming?

· Which entity was correct in their assessment of the evaluation’s research status (i.e. was this research or non-research)?
