Academic clinical history and physical notes provide a unique opportunity to practice and demonstrate advanced practice documentation skills, to develop and demonstrate critical thinking and clinical reasoning skills, and to practice identifying acute and chronic problems and formulating evidence-based plans of care.
Complete an academic clinical history and physical note based on a patient seen during clinical. In your assessment, provide the following. ( Acute Care Hospital)
History and Physical Note
1. Chief complaint/reason for admission/visit/consult.
2. HPI for the H&P or consult notes.
3. Medical, surgical, family, social, and allergy history.
4. Home medications, including dosages, route, frequency, and current medications, if a consultation note
5. Review of systems with all body systems for H&P or consult notes. Review of systems is what the patient or family/friends tell you (by body system).
6. Vital signs and weight.

Avoiding Plagiarism

APA Style and References
One thing you will need to do in college is learn how to find and use references. References support your ideas. College-level work must be supported by research. You are expected to do that for this paper. You will research, locate, and use one reputable source from the literature on the topic of epigenetics/epigenesis. The “literature” is composed of studies that have been conducted in a scholarly way to support ideas. Scholarly sources can be found in journals or in some Web sites, especially those that come from .edu domains (.edu is short for “educational” Web sites) or .gov domains (.gov are government sites). The College Libraries have dozens of good journals you can use. You will be using the College Library for this paper. You may do the research in person or use the Web to access our library. In APA style, you cite a source in two places: within the body of the paper where you use the idea or words of the author of the paper; and again in the Reference List at the end of the paper. For example, you might say, “According to Researcher A (2011)…epigenetics is…” What you must do is supply the researcher’s name within a sentence or at the end of a sentence in parenthesis (Smith, 2011). Then you give the full information for locating the study in the Reference List. That’s what Reference Lists are for: they allow the person reading your paper to look up your source if they want to. Healthy Chicago gives us a look at how all of us can have a voice in creating a healthier city. Change happens when we make healthy choices as a society. Looking at health in a broad sense is what public health is about. Everything impacts a person’s health: housing, education, income, community design, transportation, and the environment. Healthy Chicago 2025 discussed a life expectancy gap between black and white residents of almost nine years. We have to evaluate what are the reasons for this gap and how we as health care providers can assist to change it. Together, we can change the direction of health in America.

Environmentalism and Moral Concern for Animals
Many believe that we are in serious trouble today as human beings plunging headlong into a major climate crisis on planet earth.
Our course eText on Environmental Ethics states the following:
There is no denying that the global climate is changing, as the level of carbon dioxide in the atmosphere has increased during the past century. … Coastlines are crumbling as the climate changes and sea levels rise… storms are increasing in severity … the Arctic ice cap is melting… (MacKinnon, 427).
But what’s causing these troubling changes? We are. MacKinnon again:
Some skeptics dispute whether the changes are entirely man-made, but the vast majority of experts believe one of the major causes of climate change is the burning of fossil fuels … (MacKinnon, 428).
And the human disregard for nature also means disregard for all species of animals that depend on livable natural habitats. Entire species today are threatened with immanent extinction. Writing in 2016, MacKinnon says “687 animal species are listed as either endangered or threatened.” That number has risen drastically since 2016, leading some scientists to conclude that we are in the midst of a global mass extinction of animal species.

Climate Change Report from 2019 (the video), and 2) an explanation of the meaning of speciesism as Dr. Richard Ryder first used it (the quotation ). “The view that only humans are morally considered is sometimes referred to as ‘speciesism’.
