You will locate and retrieve at least 5 research studies/references that address your selected problem. Introduction: Communication Change Challenge (CCC) Part 7

In Part 7, you will finally try out your new communication behaviors in both planned and unplanned situations.

Part 7 requires up to a 2-week window of time to complete because it requires you to implement your new behaviors and to observe, record, and analyze the impact. As a result, this part was assigned during Week 6, so you need to use all conversations from Week 6 and Week 7.

For Part 7 of the project, you will do the following:

plan a minimum of 3 situations in which you will implement your new communication behavior over a time span of 2 weeks (during Weeks 6 and 7).

Maintain a dated journal narrative to describe what happened in both planned and unplanned circumstances, including your thoughts on how well you performed the behavior.

Instructions

Include the following items in Part 7 of your report:

7A. Planning

Create a list of at least 3 specific planned situations in which you will implement your new behaviors over a time span of 2 weeks, from Week 6 and Week 7. Note: You should also implement your new behaviors in any unplanned situations when you have the opportunity to do so.

7B. Evaluation of Planned Conversations

Evaluate your performance each time you attempt your new behavior in new planned situations. As you proceed with this phase of the project, be prepared for potential problems.

You may reach periodic plateaus in which you are not changing as dramatically as you would like.

You may discover that your goal statement is not accurate and that you were trying to increase the wrong behaviors.

You may also find that by merely monitoring the frequency of your goal behavior, your problem disappears.

In any case, remain flexible and keep working. If necessary, revise your goal statement, change your target behavior, and promise yourself a reward for reaching some goal. The goal is communication behavior change, and there are many avenues for achieving and maintaining that change.

Write a dated narrative journal evaluation that describes in detail what occurred upon implementation of your new communication behaviors in planned circumstances. Address the 3 or more planned opportunities as you described them in 7A. Be sure to address the following questions in your evaluation for each conversation:

Aim to explain 2 or 3 unplanned conversations. Be sure to address the following questions in your evaluation for each conversation:

Exercise 2.5  

Diabetes affects a growing number of Americans. You have been invited to join a collaborative of community agencies interested in tackling diabetes from a community perspective.  I reside in Maryland, in Howard county.

1) What resources will you use to identify different outcomes related to diabetes? 

2) What outcomes related to diabetes are of most interest to community members? 

3) How will you compare the outcomes you select to monitor at the local level with state and national outcomes?

The objectives for Week 1 are:

Integrate key terms of epidemiology and population health to the Advanced Practice Nurse role.

Recommend primary, secondary, and tertiary prevention strategies as related to the objectives of HP2020.

Plan for inter-professional collaborations aimed at achieving HP2020 goals and objectives.

Compare and contrast sources of epidemiological data that are used to inform levels of prevention.

The last weekly objective, #4, uses the word “sources”. This correlates to Exercise 2.5’s use of the word “resources”. By this it means, where can a provider find data that inform them regarding outcomes? What is this data, what are the outcomes? Consider morbidity, mortality, incidence, prevalence, cases, various cultural populations such as racial or socioeconomic information. 

Data sources include but are not limited to these examples: State level public health departments, state-level health, and human services or task forces, federal level agencies, CDC, HP2020, Office of Minority Health

What resources will you use to identify different outcomes related to diabetes? 

I think it is helpful, to begin with defining population health outcomes. The next piece is to clearly understand how these outcomes are different than individual outcomes. What terms are used to describe population health-based outcomes?

After clearly identifying those pieces, the next step is to know where a provider can find that information. Please respond to these questions in your reply post using the course textbook and Canvas readings to support the content.

Find an article in the library that is about population health outcomes. This can be outcomes for any health issue, diabetes, obesity, kidney disease, etc. It should be for the US. Please provide a link to the article in your reply along with a brief summary.

DPI Project

I need these answered in about 150 words each  

1. It is pretty much expected that a lot of information related to quality improvement or patient safety is embedded in the EHR. It is also important to remember that in the completion of your DPI project, HIPAA rules are adhered to. Can you share a little more specifically if you will access the patient’s chart in the EHR, what information you will collect in the EHR if you will access and what ethical considerations are in place to ensure you stay within regulations?

2. You make a good point that an intervention over an 8-week period might be more effective than one over a 4 week period as well as the discussion of Internet based technology interventions. For your project looking at the nurse led education intervention, how are you specifically going to use technology? Will you do these education interventions via an Internet based technology like zoom? what if a patient does not have Internet access? will you exclude those patients? One would argue that the folks who may not have Internet access are ones experiencing socio-economic difficulties which place them at an increased risk of health complications and so would be the ones needing your intervention the most. There have been many systematic reviews looking social determinants of health and their impact on health outcomes including glycemic control in type 2 diabetics and there is a clear correlation there ( Walker et al, 2014)

3. What technology will you use to gather your data and analyze it? What statistical tests will you run if any? What specific data analysis will complete once you have your data? For example, will only look at descriptive data? what specific descriptive data? will you do a t test?…using what software? If you are unsure, this might be a good time to revisit your statistics material or reach out to a statistician to review
