While the implementation plan prepares students to apply their research to the problem or issue they have identified for their capstone project change proposal, the literature review enables students to map out and move into the active planning and development stages of the project.

A literature review analyzes how current research supports the PICOT, as well as identifies what is known and what is not known in the evidence. Research shows that the U.S. spends the most money on health care compared to other high-income countries. Although the United States spends more money, United States citizens’ health quality is lower.

The United States is the lowest ranking for overall quality of health out of the eleven high income countries. Canada is ranked number 9 out of 11. The United States and Canada are similar when comparing aspects of their healthcare system. For example, both countries are ranked below average for access to health care, equity, and health care outcomes. Additionally, Canada and the United States are ranked above average for care processes related to prevention, safety, and engagement from patients and doctors. For administrative efficiency, Canada ranks above the average while the U.S. remains below average. (The Commonwealth Fund 2017)

One major difference between Canada and the United States is that Canada provides universal health care and the U.S. does not. Non-citizens living in the United States may not qualify to receive health care. As for Canada, temporary visitors and immigrants are eligible for limited health care benefits. Summary: As scientists and doctors struggle to find a cure or vaccine for COVID-19, racial and economic disparities are making the impact worse for some groups. Nearly 30 percent of U.S. patients were Black — even though African Americans make up only about 13 percent of the general population. The share of cases among Latinos is also disproportionately large. This story examines significant racial disparities that exist in our society and how they affect the pandemic’s toll. Real people and doctors discuss the complicated social and political intersections the pandemic has brought to light.

Introduction

While treating the chronically ill, a major challenge is developing a plan of care that addresses the specific needs of a patient and a caregiver. You need to be in close touch with patients and their support group, family and peers, to come up with an ideal plan.

In this course project, you are going to develop a plan of care for a chronic illness group of your choice using the Healthy People 2020 topics. While executing the tasks of this project, remember that while you need to give a general overview of the biomedical considerations of the case, the focus should always be on the psychosocial elements. Your perspective in this care plan should be the patient’s goals rather than those of the medical team.

Each week, you will complete a part of this project, submitting the final project in Week 5.

Identifying a Group

Identify a chronic illness of specific interest to you that is also identified as a Healthy People 2020 topic (healthypeople.gov). In a Microsoft Word document of 4-5 pages formatted in APA style, address the following criteria:

·  Identify a chronic illness and rationale for choice.

·  Develop a questionnaire by utilizing your knowledge of health and illness, with the aim of acquiring all information you need from patients to prepare a plan of care for the specific illness group.

·  Discuss morbidity and comorbidity of the disease.
