CLC PowerPoint W5

This is a Collaborative Learning Community (CLC) assignment.

Some patients will seek out alternative medicinal practices when treating diseases; therefore, it is important to know how alternative or herbal medications interact with prescription medications. Knowing how to navigate finding information on potential interactions is important in preventing complications when prescribing medications.

Groups will select a common disease and perform research on both conventional and alternative medicine treatment options. Using CAM as your primary resource, create a 8-10 slide PowerPoint presentation that addresses the following items:

1. Identify the disease.

2. Identify the patient population that is most affected by this disease.

3. Identify complementary and alternative medicine resources and guidelines.

4. Evaluate the safety and effectiveness of complementary and alternative therapies.

5. Identify any common interactions of alternative and nonpharmaceutical medications with prescribed medications.

Include slide notes (150-250 words for each slide).

You are required to cite Four to five sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.

NRS-410V-0L191 Pathophisiology and Nursing Management of Client Health.

Case Study: Mrs. J.

It is necessary for an RN-BSN-prepared nurse to demonstrate an enhanced understanding of the pathophysiological processes of disease, the clinical manifestations and treatment protocols, and how they affect clients across the life span.

Evaluate the Health History and Medical Information for Mrs. J., presented below.

Based on this information, formulate a conclusion based on your evaluation, and complete the Critical Thinking Essay assignment, as instructed below.

Health History and Medical Information

Health History

Mrs. J. is a 63-year-old married woman who has a history of hypertension, chronic heart failure, and chronic obstructive pulmonary disease (COPD). Despite requiring 2L of oxygen/nasal cannula at home during activity, she continues to smoke two packs of cigarettes a day and has done so for 40 years. Three days ago, she had sudden onset of flu-like symptoms including fever, productive cough, nausea, and malaise. Over the past 3 days, she has been unable to perform ADLs and has required assistance in walking short distances. She has not taken her antihypertensive medications or medications to control her heart failure for 3 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure and acute exacerbation of COPD.

Subjective Data

  1. Is very anxious and asks whether she is going to die.
  2. Denies pain but says she feels like she cannot get enough air.
  3. Says her heart feels like it is “running away.”
  4. Reports that she is exhausted and cannot eat or drink by herself.

Objective Data

  1. Height 175 cm; Weight 95.5kg.
  2. Vital signs: T 37.6C, HR 118 and irregular, RR 34, BP 90/58.
  3. Cardiovascular: Distant S1, S2, S3 present; PMI at sixth ICS and faint: all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring indicates a ventricular rate of 132 and atrial fibrillation.
  4. Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%.
  5. Gastrointestinal: BS present: hepatomegaly 4cm below costal margin.

Intervention

The following medications administered through drug therapy control her symptoms:

  1. IV furosemide (Lasix)
  2. Enalapril (Vasotec)
  3. Metoprolol (Lopressor)
  4. IV morphine sulphate (Morphine)
  5. Inhaled short-acting bronchodilator (ProAir HFA)
  6. Inhaled corticosteroid (Flovent HFA)
  7. Oxygen delivered at 2L/ NC

Critical Thinking Essay

In 750-1,000 words, critically evaluate Mrs. J.’s situation. Include the following:

  1. Describe the clinical manifestations present in Mrs. J.
  2. Discuss whether the nursing interventions at the time of her admissions were appropriate for Mrs. J. and explain the rationale for each of the medications listed.
  3. Describe four cardiovascular conditions that may lead to heart failure and what can be done in the form of medical/nursing interventions to prevent the development of heart failure in each condition.
  4. Taking into consideration the fact that most mature adults take at least six prescription medications, discuss four nursing interventions that can help prevent problems caused by multiple drug interactions in older patients. Provide a rationale for each of the interventions you recommend.
  5. Provide a health promotion and restoration teaching plan for Mrs. J., including multidisciplinary resources for rehabilitation and any modifications that may be needed. Explain how the rehabilitation resources and modifications will assist the patients’ transition to independence.
  6. Describe a method for providing education for Mrs. J. regarding medications that need to be maintained to prevent future hospital admission. Provide rationale.
  7. Outline COPD triggers that can increase exacerbation frequency, resulting in return visits. Considering Mrs. J.’s current and long-term tobacco use, discuss what options for smoking cessation should be offered.

You are required to cite to a minimum of two sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and relevant to nursing practice.

Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Ethics and Evidence-Based Research

Write a 1250-1500 word essay addressing each of the following points/questions. Be sure to completely answer all the questions for each bullet point. There should be three main sections, one for each bullet below. Separate each section in your paper with a clear heading that allows your professor to know which bullet you are addressing in that section of your paper. Support your ideas with at least two (2) sources using citations in your essay. Make sure to cite using the APA writing style for the essay. The cover page and reference page in correct APA do not count towards the minimum word amount. Review the rubric criteria for this assignment.

Part 1: Describe why ethical safeguards designed for clinical research may not be feasible or appropriate for evidence-based practice or evidence-based practice implementation projects.

Part 2:  Review the sectioned headed, Two Ethical Exemplars in Chapter 22 of the textbook (Melnyk and Fineout-Overholt, 2015, pages 518-519). Most healthcare treatment options are voluntary, meaning that a patient has the right to refuse care, even if it is in their best interest to comply. However, in some cases, psychiatric treatment can be mandatory. Involuntary treatment (which is also referred to as “assisted treatment”) is typically implemented when patients have been diagnosed with a severe mental disorder and pose a danger to themselves or others. Although the stipulations vary by state, all fifty states allow for some form of involuntary treatment for mental illness. Be sure to support your answers to the following questions with research.

  • Is involuntary care ethical?
  • Do patients with severe mental illness have the mental capacity to refuse or consent to treatment?
  • Should patients be forced to take medications, be hospitalized, or undergo therapy against their will? If so, what should be the consequence for refusal?
  • Are there better ways to encourage patients to adhere to their treatment plan?

HOMEWORK ASSIGNMENT

The WHO has published an important document titled, “Social Determinants of Health—The Solid Facts.” It states:

Even in the most affluent countries, people who are less well off have substantially shorter life expectancies and more illnesses than the rich. Not only are these differences in health an important social injustice, they have also drawn scientific attention to some of the most powerful determinants of health standards in modern societies. They have led in particular to a growing understanding of the remarkable sensitivity of health to the social environment and to what have become known as the social determinants of health.

http://www.euro.who.int/__data/assets/pdf_file/0005/98438/e81384.pdf

1. In your opinion, are social determinants of health more important for a health educator to understand than the biological/genetic determinants of health?

2. Additionally, if the social determinants of health are so critical to one’s health outcomes how can health education improve the social environment/condition of persons most affected by social determinants? Please post your response by the end of this module and critique the response of at least one other student. Please provide at least two paragraphs for your response.
