 Consider the following quote from chapter 14 of the eText: ”The gap between the rich and poor in the United States …has been widening for some time. In recent years, the gap has been the widest since our government began to keep records on it in 1947” (eText, p.143).  For example, one report states that: “Since 2009, 95% of U.S. economic gains have gone to the wealthiest 1% of the population” (“Income Inequality: Why Does the Gap Keep Widening?” The Week Feb. 7, 2014).

But not only is there a widening gap between the “Rich and the Rest of Us” (to use the title of a recent book by Tavis Smiley and Cornel West: LINK (Links to an external site.)), there is also the ever widening reach of marketing into everything or what Michael Sandel calls the marketization of society.  In Sandel’s TED Talk video, he expresses this concern that America is becoming a market society, meaning a society that uses monetary rewards to motivate desired behaviors in all aspects of life (LINK (Links to an external site.)).

For our discussion, please answer the following two questions:

[1] Discuss the growing income inequality in the U.S. and the world with someone mature enough to have some perspective from experience about it.  Ask them what they think about the widening gap between the rich and the rest of us in America, as our course materials speak of this problem.  How has the economic trend for middle class Americans affected them?  How as it affected you?

[2] Two examples of the marketization of society today that Sandel discusses are cash for grades and pay for getting around long lines.  What, if anything, is wrong with these practices?  Beyond what Sandel discusses, think of at least one social, personal, or relational good or value that you think would be negatively affected by marketization.  And explain why.

Role of the Family Nurse Practitioners (FNPs) consists of dealing with 45% acute health problems, 34% episodes of exacerbation of chronic conditions and 24.5% for non-illness related health promotion visits. Collaboration with supervised Physician and signed collaborative agreement must be in place. Data suggests that mid level health professionals provide counseling in 84%, where Physicians counsel in 61% of visits. Advance Care Providers not only share the patient’s load, but also ensure superior patient satisfaction by providing non-billable services before, during, and after treatment (Pickard, 2014 p.130).

Non-billable value-adding activities comprise of educating patients, monitoring treatment care, following patients from the beginning to the end, managing adverse events/reactions, coordinating referrals to other specialist and keeping communication with them, preparing for procedures and executing them, answering questions, and handling complicated cases (Yopp, Wall, & Miller, 2016, p. 749).    

          Nurse practitioners (NPs) are required to follow state’s regulations at their work place. Medications of schedule II, III, IV, and V are considered to be controlled substances, consequently NPs are allowed to prescribe them under the supervision of a collaborating medical doctor (MD). Floridian NPs, who undergo proper training and have appropriate experience, can receive a federal waiver to dispense buprenorphine-containing products, as long as the supervising Physician is certified, trained, or permitted to treat and manage patients with opioid use disorder (Florida Scope of Practice Policy – State Profile).

The term “knowledge worker” was first coined by management consultant and author Peter Drucker in his book, The Landmarks of Tomorrow (1959). Drucker defined knowledge workers as high-level workers who apply theoretical and analytical knowledge, acquired through formal training, to develop products and services. Does this sound familiar?

Nurses are very much knowledge workers. What has changed since Drucker’s time are the ways that knowledge can be acquired. The volume of data that can now be generated and the tools used to access this data have evolved significantly in recent years and helped healthcare professionals (among many others) to assume the role of knowledge worker in new and powerful ways.

In this Assignment, you will consider the evolving role of the nurse leader and how this evolution has led nurse leaders to assume the role of knowledge worker. Medicaid Expansion

Scenario

You are the director of community affairs for the health lobby organization, Pathways to a Healthy America. In 2010, President Barack Obama signed the Affordable Care Act into law. As a result, millions of Americans who were previously uninsured gained access to the healthcare system. One of the initial provisions of the Act required individual states to expand the eligibility criteria for Medicaid. The program’s costs would be absorbed by the federal government at a rate of 100% for the first three years of program participation which would decrease to 90% after 2020, still, considerably higher than previous funding by the federal government. However, in 2012, the Supreme Court ruled the mandate unconstitutional allowing individual states to voluntarily “opt” out of the Medicaid expansion program. As a result of the various states’ decision to “opt” out of the ACA’s Medicaid expansion, millions of adults fell into the critical coverage gap. Your organization represents a state which has decided to forego participation in the ACA expansion program. The decision was met with both praise and criticism.

It has now been a few years since the decision and your organization has gathered information on the impact of the decision in order to advocate for Medicaid reform during the next state legislative session.

You are required to write an advocacy report to state lawmakers in which you advocate for either participating in the original expansion program or participating with conditions (waivers) to address the critical gap in coverage for vulnerable adults in your state. Waivers such as Section 1115 enable for alternative implementation of Medicaid expansion and allow states to impose restrictions which may result in a denial of Medicaid eligibility for individuals who do not meet those restrictions, i.e., work requirements.
