Write a 1- to 2-page summary paper that addresses the following:

• Briefly summarize the patient case study, including each of the three decisions you chose for the patient presented. Support your decisions with evidence-based literature. Be specific and provide examples.
• What were you hoping to achieve with the decisions you recommended for the patient case study? Support your response with evidence and references from outside resources.
• Explain any difference between what you expected to achieve with each of the decisions and the results of the decision in the exercise. Describe whether they were different. Be specific and provide examples.

• What can happen with the implementation of a new process if a workflow has not been planned? 
• How can a workflow diagram help to train new staff members, or help seasoned staff members with a change in their job?

• Review the Patient Portal Snapshot Scenario. Select one patient for your assessment. Prepare an individualized patient-focused handout for that patient that incorporates care coordination. For the video component of the assessment, you can use the Brightspace video tool or submit a .mp4. Please note the Brightspace video tool is limited to 3 minutes but the video can be submitted in 2 parts.

As a nurse at Memorial Hospital–a large city hospital, you provide care to diverse patients. As you work the nursing units at the hospital, you engage with the electronic health records of hundreds of patients each day as a way of accessing, recording, and developing guidelines for safe and quality patient care. Today, you have accessed a patient portal to help you develop and design educational materials for your patient. As you review the pertinent information in the patient portal, you begin considering how you will communicate these educational materials to your patient. After gathering your thoughts and obtaining the necessary history and medical needs of your patient, you are ready to create and deliver this educational plan to your patient.

This assessment has two-parts.  Click each of the items below to complete this assessment.

Part I: Educational Plan

Design and create an educational plan (brochure or pamphlet) that addresses the following:

1. Include relevant health information necessary to educate your patient on their healthcare needs.
   1. Describe and define the health diagnoses, treatments, and at least one medication listed in the patient portal
   2. Explain relevant statistics and individual health vital signs related to the patient’s medical history
   3. Explain additional sources of support for patients with similar health needs/diagnoses
      1. Consider the context of your individual patient, and be sure to include any recommendations on how to access sources of support based on racial/ethnic background or culture, socioeconomic status, educational background, health literacy and linguistic proficiency
2. Be sure to adhere to health literacy guidelines for your individual patient.
3. Make sure to address each of the six areas from the “Checklist for Easy-to Understand Print Materials”
4. Your educational plan should be visually appealing and relevant to your individual patient.
5. Be sure to support the development of your educational plan by consulting and referencing 3- 5 scholarly references.
6. Length of the educational plan: 1- to 2-pages; include a separate page for you reference list.

Part II: Delivering Your Educational Plan

When delivering your educational plan to your patient, include the following:

1. Record a 3- to 5-minute video presentation detailing how you would communicate this educational plan to your patient.
2. Be sure to incorporate patient-centered care strategies in the design and delivery of your educational plan.
   1. Be sure to not just read the educational plan you created, but rather, consider how you would engage with your patient based on health literacy, culture, and background when educating your patient based on their health needs
3. Include a plan for care coordination.
   1. Explain your plan for care coordination based on the educational plan you developed.
4. Be sure to speak clearly and confidently when presenting your educational plan to your patient.

Using resources such as the CDC, state and county health department websites, and various local resources, identify an opportunity for improved health behavior in an individual client. Your paper should be 1 – 2 pages in length in addition to the cover page and reference page. You should use a minimum of three references to support your work.

Be sure to cover the following:

• Identify an opportunity to for improved health behavior in an individualized client.
• Identify short and long term goals for the achievement of the overall health goal.
• Using Healthy People 2020, identify which problem you are working on improving.
• Recognize local resources to aid in the resolution of the identified problem.
• Assist in the evaluation in the response to interventions.

Apply information from the Aquifer Case Study to answer the following questions:

• Discuss the history that you would take on this child in preparation for the well-child visit. Include questions regarding her growth and development that are appropriate for her age.
• Describe the developmental tool to be used for Asia, its reliability and validity and how Asia scored developmentally on this tool. Is she developmentally appropriate for her age?
• What immunizations will Asia be given at this visit; what is the patient education and follow-up?

Nursing models provide various perspectives for thinking critically and making nursing decisions.

Choose a nursing model and identify the concepts, values, and beliefs as they apply to contemporary nursing practice.
