Assessment/Interview

Select a community of interest in your region. Perform a physical assessment of the community.

1. Perform a direct assessment of a community of interest using the “Functional Health Patterns Community Assessment Guide.”
2. Interview a community health and public health provider regarding that person’s role and experiences within the community.

Interview Guidelines

Interviews can take place in-person, by phone, or by Skype.

Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.

Complete the “Provider Interview Acknowledgement Form” prior to conducting the interview. Submit this document separately in its respective drop box.

Compile key findings from the interview, including the interview questions used, and submit these with the presentation.

PowerPoint Presentation

Create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.

Include the following in your presentation:

1. Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.
2. Summary of community assessment: (a) funding sources and (b) partnerships.
3. Summary of interview with community health/public health provider.
4. Identification of an issue that is lacking or an opportunity for health promotion.
5. A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.

While APA style is not required for the body of this assignment, solid academic writing is expected, and documentation of sources should be presented using APA format ting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

Everyone should have an approved topic for your final paper on an ethical dilemma of your choosing. This week’s assignment is to write the first, and most important, part of your paper (my topic “Incompetent/Impaired Nurses”)

2. What is the scope, context, and significance of the topic as an ethical dilemma?
3. Highlight the importance of the problem/issue and make general statements about the problem/issue
4. Summarize the current understanding and background information about the topic
5. Include one reference source that will support your introductory thoughts about your topic.
6. APA 7th edition format; full sentences, attention to grammar

A 32-year-old female presents to the ED with a chief complaint of fever, chills, nausea, vomiting, and vaginal discharge. She states these symptoms started about 3 days ago, but she thought she had the flu. She has begun to have LLQ pain and notes bilateral lower back pain. She denies dysuria, foul-smelling urine, or frequency. States she is married and has sexual intercourse with her husband. PMH negative.

Labs: CBC-WBC 18, Hgb 16, Hct 44, Plat 325, ­ Neuts & Lymphs, sed rate 46 mm/hr, C-reactive protein 67 mg/L CMP wnl

Vital signs T 103.2 F Pulse 120 Resp 22 and PaO2

99% on room air. Cardio-respiratory exam WNL with the exception of tachycardia but no murmurs, rubs, clicks, or gallops. Abdominal exam + for LLQ pain on deep palpation but no rebound or rigidity. Pelvic exam demonstrates copious foul-smelling green drainage with reddened cervix and + bilateral adenexal tenderness. + chandelier sign. Wet prep in ER + clue cells and gram stain in ER + gram negative diplococci.

Develop a 1- to 2-page case study analysis, examining the patient symptoms presented in the case study. Be sure to address the following as it relates to the case you were assigned (omit section that does not pertain to your case, faculty will give full points for that section).

The sections that you are to omit are for the above case study are: 1. Explain why prostatitis and infection happen. Also explain the causes of systemic reaction, 2. Explain why a patient would need a splenectomy after a diagnosis of ITP, and 3. Explain anemia and the different kinds of anemia (i.e., micro and macrocytic).

In your Case Study Analysis related to the scenario provided, explain the following:

• The factors that affect fertility (STDs).
• Why inflammatory markers rise in STD/PID.
• Why prostatitis and infection happens. Also explain the causes of systemic reaction.
• Why a patient would need a splenectomy after a diagnosis of ITP.
• Anemia and the different kinds of anemia (i.e., micro and macrocytic).

Additionally, describe the role of the National Organization for Human Service Education and the Council for Standards in Human Service Education in their application.  

Your assignment should be two- to- three pages (excluding title and reference pages).  Include at least two references to online sources and at least one peer-reviewed journal article that was published within the last five years found at the Ashford online library.  All sources must be formatted according to APA guidelines as outlined in the Ashford University Library.

2- 300 minimum words for every DQ, you can go up to 800 words but answer should be complete.

3- 2-3 Peer Reviewed/ scholarly references for each question

6- The response to the DQ is expected to be a minimum of 300 words. A minimum of two peer reviewed/ scholarly  resources are expected. These need to be appropriate for a clinical professional to guide decisions about patient care. If a textbook is used for one of these responses, the other needs to be journal or professional-level website. The references need to be correctly formatted, as do the citations for those references.  “ Question words” don’t count towards 300 minimum count”

Asking the Relevant Clinical Questions

Evidence-based nursing is a process founded on the collection, interpretation, appraisal, and integration of valid, clinically significant, and applicable research. It is not about developing new knowledge or validating existing knowledge, but rather translating existing evidence so that it can be applied to clinical decision making. This process starts with formulating a sound PICO question. PICO is a format for developing a good clinical research question prior to starting a review or research. It is a mnemonic used to describe the four elements of a sound clinical foreground question.

Scenarios:

#1   A nurse in the PACU is interested in ways to decrease the use of pain medication in post-op patients. She’s wondering if classical music or guided imagery might be efficacious.

#2   A 17-year-old African American woman, just diagnosed with hepatitis B, has been taking acetaminophen for headaches. Her mother has heard that acetaminophen can cause liver problems, and she asks you if her daughter should continue taking the drug for her headaches, considering her hep-b diagnosis.

#3   You are on a hospital committee exploring best practices in inpatient oncology nursing. The first question to come before the committee is, “how can we improve patients’ sleep quality and quantity without resorting to sleep medication?” The committee decides to explore relaxation techniques (e.g., massage; music), noise reduction, bed/pillow comfort, and ambient temperature changes. Your assignment is to find the best practices to reduce noise levels on the unit.

#4   A 53-year-old woman with rheumatoid arthritis wants to possibly change her medication to short-term low-dose corticosteroids instead of NSAIDS, which are upsetting her stomach. Will the corticosteroids be as effective?

#5   Jeff, a smoker of more than 30 years, has tried to quit unsuccessfully in the past. A friend of his recently quit with acupuncture. He wonders if he should try it.

Instructions:

1. Choose one (1) clinical scenario above, and answer the following questions:
   1. What is evidence-based nursing practice and why is it important?
   2. What is the PICO process?
   3. Formulate a well defined clinical question that will yield the most relevant and best evidence.
   4. Describe the background of the question
   5. Present the question using PICOT format
   6. Indicate what will be the best study to answer the clinical question.

This week you have learned about common cardiovascular disorders in the Adult and Geriatric patient. For the purpose of this discussion select one of the following cardiovascular disorders and provide the following in your initial post:

• Common Signs and symptoms seen
• Screening assessment tools
• Recommended diagnostic tests (if any)
• Treatment plans both pharmacologic and non-pharmacologic based on current clinical practice guidelines

Although a seemingly basic question, the difference between a dataset and a database has important implications for how data is applied in practice—how it is viewed, extracted, and importantly for the nurse informaticist, how it is exchanged. In this Discussion, you examine this difference.

This Discussion has two components. The first component prompts you to consider different types of datasets or databases within professional practice. 

The second component aligns with the Assignment in this module, where you will interview a professional nurse informaticist. In this component, you post draft questions in the Discussion for feedback from your colleagues.

To Prepare

· Review the Resources and consider the differences between datasets and databases.

· Reflect on the types of data obtained and how they are used in sharing across health information systems.

· Review the Module 2 Assignment.

· Review the requirements of the Assignment and the guidelines for developing interview questions.

· Develop a set of draft questions to post as the second component of your Discussion.

Post a response explaining the differences between datasets and databases. Explain how you might use each type of data in your professional practice. Be specific and provide examples. Then, post the questions you plan to ask your interviewee during your scheduled interview for your module Assignment.
