Download the SOAP template to help you design a holistic patient care plan. Utilize the SOAP guidelines to assist you in creating your SOAP note and building your plan of care. You are expected to develop a comprehensive SOAP note based on the given assessment, diagnosis, and advanced nursing interventions. Reflect on what you have learned about care plans through independent research and peer discussions and incorporate the knowledge that you have gained into your patient’s care plan.  If the information is not in the provided scenario please consider it normal for SOAP note purposes, if it is abnormal please utilize what you know about the disease process and write what you would expect in the subjective and objective areas of your note.

Format

• Your care plan should be formatted as a Microsoft Word document. Follow the current APA edition style. Your paper should be no longer than 3-4 pages excluding the title and the references and in 12pt font.
• Name your document: SU_NSG6001_W4A2_LastName_FirstInitial.doc.
• Submit your document to the Submissions Area by the due date assigned.

 Your friend Bruce calls in a panic. He has just come from the doctor and been diagnosed with a bacterial infection. The doctor told him not to worry; his B cells will take care of it in no time! Knowing that you are studying the immune response, he is calling to ask you to explain this statement. Is the doctor correct? Is there more to the story? 

Prepare a concept map for a musculoskeletal disorder from your readings. Use the included template to outline the system disorder including the pathophysiology, etiology, clinical manifestations, and treatment.

Use at least one scholarly source to support your findings. Examples of scholarly sources include academic journals, textbooks, reference texts, and CINAHL nursing guides. Be sure to cite your sources in-text and on a References page using APA format.

Go through and put in paragraphs for each of your sections. You have long blocks of content that need to be broken up for better organization. This demonstrates that you know how to organize content and makes it easier for the reader to follow your train of thought. Also make sure that the economic burden section follows the formatting of the template. You need to separate out different parts of the economy that are affected.

When you first went to work for your current organization, experienced colleagues may have shared with you details about processes and procedures. Perhaps you even attended an orientation session to brief you on these matters. As a “rookie,” you likely kept the nature of your questions to those with answers that would best help you perform your new role.

Over time and with experience, perhaps you recognized aspects of these processes and procedures that you wanted to question further. This is the realm of clinical inquiry.

Clinical inquiry is the practice of asking questions about clinical practice. To continuously improve patient care, all nurses should consistently use clinical inquiry to question why they are doing something the way they are doing it. Do they know why it is done this way, or is it just because we have always done it this way? Is it a common practice or a best practice?

In this Assignment, you will identify clinical areas of interest and inquiry and practice searching for research in support of maintaining or changing these practices. You will also analyze this research to compare research methodologies employed.

To Prepare:

• Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry. Keep in mind that the clinical issue you identify for your research will stay the same for the entire course. 
• Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least four different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles.
• Review the results of your peer-reviewed research and reflect on the process of using an unfiltered database to search for peer-reviewed research.
• Reflect on the types of research methodologies contained in the four relevant peer-reviewed articles you selected.

Part 1: Identifying Research Methodologies

After reading each of the four peer-reviewed articles you selected, use the Matrix Worksheet template to analyze the methodologies applied in each of the four peer-reviewed articles. Your analysis should include the following:

• The full citation of each peer-reviewed article in APA format.
• A brief (1-paragraph) statement explaining why you chose this peer-reviewed article and/or how it relates to your clinical issue of interest, including a brief explanation of the ethics of research related to your clinical issue of interest.
• A brief (1-2 paragraph) description of the aims of the research of each peer-reviewed article.
• A brief (1-2 paragraph) description of the research methodology used. Be sure to identify if the methodology used was qualitative, quantitative, or a mixed-methods approach. Be specific.
• A brief (1- to 2-paragraph) description of the strengths of each of the research methodologies used, including reliability and validity of how the methodology was applied in each of the peer-reviewed articles you selected.

Assignment:

This week students will be addressing the following essay questions. Insure all aspects of the questions are addressed. Submission needs to be in APA format. Use a Level 1 heading to separate your sections (Page 62 of the APA Publication Manual).

The home and community are increasing in importance as the recommended point of care delivery.  Identify a recent news article that supports this assertion. Summarize the article and discuss how this information will influence and support the patient and family in achieving the best possible outcomes while remaining in the community. Provide a link to the article you have chosen for this discussion post.

Your initial posting should be 250 to 350 words in length and utilize at least one scholarly source other than the textbook.
