Cultural diversity refers to the differences among people based upon shared ideology and shared valued sets of beliefs, norms, customs, and meanings evidenced in a way of life. This diversity is expressed in many ways. Diversity in history, beliefs, practices, and opportunities not only exist among the many cultural groups, they typically exist across a wide continuum within the group. Health behaviors are influenced by culture and cultural values as well as socioeconomic status. In this assignment, students will develop a health promotion project for a cultural group living anywhere in the United States. You are to learn as much as you can about the culture norms and patterns and how these norms and patterns influence health practices. The purpose of this assignment is to develop a deeper understanding and sensitivity regarding the role of culture in patients’ lives and in nursing through the examination of a cultural/ethnic group while addressing health promotion strategies and disease prevention. PROJECT COMPONENTS The project should address the following Aspects of Cultural Assessment: 1. Ethnic/racial identity: How does the group identify itself in terms ethnicity and racial background? What is the range of interaction outside of the cultural group? Are recreational, educational, and other social activities within the ethnic reference group, the wider community, or both? 2. History and Value Orientation: How are values derived? What is the historical experience of the group that may have impacted values? 3. Language-communication process: What is the primary spoken language? What languages are spoken in the home? And by whom? What language is preferred when speaking to outsiders? Do second and third generations in the speak the language of their grandparents? 4. Health Beliefs and Practices: What are the traditional health beliefs? How common are these beliefs and practices within this group? To what extent are folk healing practices and practitioners used? 5. Religious Belief and Spirituality: Discuss religious articles & practices. How is spirituality is defined in the culture? Remember that Religious Beliefs and Spirituality are different concepts. 6. Lifecycle events: Which life cycle events are important to the culture? What are the customs associated with births, coming of age, marriage, and death? 7. Nutritional Behavior/Diet: Are there restrictions? Are there common practices? 8. Medical health care access and experience: Is access to care and equality of care an issue for this group? For example, are group members less likely to receive needed care due to system issues such as financial access or provider biases than another group? Based on the Cultural Assessment Data, the group will design a Cultural Health Promotion Plan: 9. Cultural Health Promotion Plan: Identify a health risk or health problem the project will address. Describe why this is an issue for your chosen target group and how cultural plays a role in the health risk or problem. 10.Health Promotion Strategies/Methods: Based on the health risk/problem identified, describe the strategies/methods that you will implement. Focus on primary and secondary prevention strategies. Refer to Healthy People 2020 Topics and Objectives. State how you will measure/assess the effectiveness of the project. This is a GROUP assignment. Groups will consist of no more than three students. A group leader must be assigned. Submit the paper via Moodle designated assignment link. Only one submission per group. DUE DATE: Week 12 Complete the paper according to APA 6th Edition Guidelines as a word-processed document. The paper is limited to a MAXIMUM of Ten (10) pages (including the title page and references pages; NO Abstract is needed for this assignment). Do not exceed the ten-page limit as only ten pages will be read and graded. The paper must be double spaced using Font Size 12 (including the reference page). PRESENTATION In addition to the paper, the group will prepare a presentation. The presentation will be graded separately – refer to course syllabus. You are the manager of your local regional health clinic. You have a meeting with your Board of Trustees coming up in a few days. Your trustees have asked you to prepare a short presentation in response to an incident that occurred at your clinic just a few weeks ago:
A young Native American woman fell sustaining an injuring to her arm. After a week, her arm is still painful which causes her to visit a physician at your clinic. The physician orders tests, ascertains that there is no fracture, and decides to simply advise over-the-counter medication for inflammation and pain. Two weeks later the patient returns with the same issue at which time the physician believes that physical therapy will help the injured arm. While undergoing physical therapy, the patient also visits with a local healer who prescribes herbal treatments for the injury. The patient returns to see the physician after two weeks with an improvement in her injury. On her last visit she wanted to discuss her desire to use natural treatments in conjunction with physical therapy with the white doctor but did not. In fact, she has discussed very little with her doctor about her injury, how it has affected her ability to care for her children and elderly mother, etc. She does not fully trust the white doctor and has heard him sound skeptical concerning healers from her culture or other types of “non-traditional” treatments. The patient does, however, feel that natural remedies administered by healers help her.

The relationships between drugs and the body can be described by pharmacokinetics and pharmacodynamics.
Pharmacokinetics describes what the body does to the drug through absorption, distribution, metabolism, and excretion, whereas pharmacodynamics describes what the drug does to the body.

When selecting drugs and determining dosages for patients, it is essential to consider individual patient factors that might impact the patient’s pharmacokinetic and pharmacodynamic processes. These patient factors include genetics, gender, ethnicity, age, behavior (i.e., diet, nutrition, smoking, alcohol, illicit drug abuse), and/or pathophysiological changes due to disease.
For this Discussion, you reflect on a case from your past clinical experiences and consider how a patient’s pharmacokinetic and pharmacodynamic processes may alter his or her response to a drug.

Pharmacology is the study of the interactions between drugs and the body. The two broad divisions of pharmacokinetics refers to the movement of drugs through the body, whereas pharmacodynamics refers to the body’s biological response to drugs. Pharmacokinetics describes the drug’s exposure by characterizing absorption, distribution, bioavailability, metabolism, and excretion as a function of time, while pharmacodynamics describes drug response in terms of biochemical or molecular interactions (Arcangelo et al., 2017). The focus of this discussion will be in the process of warfarin in term of pharmacokinetics and pharmacodynamics of it in the body. Ms. J.J. ‘s Health issue: I was in charge of Ms. J. care couple years ago, a 85 year old African American women who was diagnosed with dementia cerebral infarction due to unspecified occlusion of cerebral artery, arthropathy, major depressive disorder, atrial fibrillation with a history of long-term use of anticoagulants, contracture of muscle, constipation, hypertension and GERD. She is currently on coumadin for the atrial fibrillation and the blood levels are monitoring every week in order to control the drugs therapeutic levels and avoid any adverse reactions. Pharmacodynamics versus pharmacokinetics of this anticoagulant: Many statistics from the stroke prevention in atrial fibrillation (SPAF) trial suggest that safety of anticoagulant in the elderly can be maximized through a careful monitoring and maintenance of the INR which is between 2 and 3. Ms. J’s therapeutic window for warfarin 2 to 3 which is the normal range for coumadin therapeutic level. Her weekly dosage is adjusted to her current blood levels. Bleeding is the most related complication of anticoagulant. Amy INR that increasing to 3.4 or 4.0 from Ms. J will result in nose bleeding, decreasing the coumadin or stop it for one or two days will be the only option (Horton & Bushwick, 1999). Factors influencing Ms. J’s drugs therapy: Multiple factors may affect the absorption of her medication. For example, the presence or the absence of flood in the stomach, blood flow to the area for absorption, and the dosage form of the drug. In Ms. J’s case, the most critical factor. Influencing her absorption of coumadin is gastric motility due to the history of constipation that she has, while a routine laxative dose and stools softens are administered daily for bowel movement. Patient-centered care plan for management of constipation: A non-pharmacologic care plan management can be introduced for the constipation in order to reduce the frequency and the quantity of laxative and stool. Softens doses that Ms. J is getting and ultimately gain a net decrease in gastrointestinal absorption of coumadin.
