Evidence based practice

As you think back over the last seven weeks, you have all come a long way with understanding research and EBP. The week introduction stated it best: “Gone is the reliance on traditions, myths, and outdated resources.”

Think about nursing practice and describe one barrier that you feel prohibits nurses from engaging in EBP; share one strategy you could use to facilitate the use of evidence to help improve nursing care for patients.

Reflect back over your time in the course and describe how your thinking has changed about nursing research and evidence-based practice; describe one new learning during this course that you believe was the most helpful.

Reflect upon the selected national practice problem in Week 1 (cancer) to address the following.

1-  From a local perspective (Miami, Florida) how does the practice problem (cancer) impact nurses, nursing care, healthcare organizations, and the quality of care being provided?

2- Identify the local key stakeholders related to the selected practice problem (cancer). *(stakeholders—healthcare providers, educators, business leaders, social service providers, community organizations, and clergy)

3- Describe one approach used in your unique setting to address this problem (cancer). From your perspective, is this intervention effective in addressing the problem? Why or why not? If this practice problem is not addressed at your workplace, propose an intervention that could be implemented on a local scale to address the problem.

 In the state of Florida in 2019, lung cancer has been rated second, on the 10 top causes of the total number of deaths and percent change 2009–2019, all ages combined.

Case Study

 Step 1: Using the following Case Study, create a meal plan.

Jackie is a healthy women.  She is 45 years old and 5’7″ tall.  Jackie’s current weight is 160 lbs.  She exercises 4-5 times per week but is struggling to meet her goal weight.  Jackie is seeking your clinical advice to lose 15 lbs in 3 months.  Her target weight goal is 145 lbs. 

Write a basic meal plan for your client. (At least 4 days of food plan to help Jackie get started)

  • Include how many calories each day she will need to consume to lose 1-2 lbs each week. I expect actual numbers you can back up (what are the specific numbers for our client Jackie? I have attached some help) 
  • Calculate her complete macro-nutrient breakdown including; total carb consumption, total fat consumption, total protein consumption
  • Explain the rationale for all of your choices (The chapter 10 slides will help especially 21-34) What is her energy requirement to stay the same (EER)?.  How many calories does she have to reduce to get to her goal? Please remember Jackie is your client not a dietitian or nutritionist.

In the media introduction to this module, it was suggested that you as a nurse have an important role in the Systems Development Life Cycle (SDLC). With a focus on patient care and outcomes, nurses may not always see themselves as contributors to the development of new systems. However, as you may have observed in your own experience, exclusion of nurse contributions when implementing systems can have dire consequences.

In this Discussion, you will consider the role you might play in systems development and the ramifications of not being an active participant in systems development.

To Prepare:

  • Review the steps of the Systems Development Life Cycle (SDLC) as presented in the Resources.
  • Reflect on your own healthcare organization and consider any steps your healthcare organization goes through when purchasing and implementing a new health information technology system.
  • Consider what a nurse might contribute to decisions made at each stage of the SDLC when planning for new health information technology.

Write a description of what you believe to be the consequences of a healthcare organization not involving nurses in each stage of the SDLC when purchasing and implementing a new health information technology system. Provide specific examples of potential issues at each stage of the SDLC and explain how the inclusion of nurses may help address these issues. Clinical significance and statistical significance are both important when determining evidence-based practice (EBP) projects. They both have their own importance, which makes them superior in their own way. Statistical significance help clinicians answer questions about probability in which they can accept or reject a hypothesis. The statisitical significance can only adress if there are statisitical differences between variables. Clinical significance requires clinicians to focus on significant changes within the clinical setting. Statistical significance helps clinicians determine that there is a problem by data collection, clinical significance helps determine if the intervention orquality initiative change project was successful and efficient.  

For my capstone project, I will use clinical significance to support positive outcomes in project by using it to determine the efficacy and efficiency of the sepsis handoff communication tool. This tool, per the EBP journal articles I have chosen for this project, shows that implementing a handoff communication tool in not only patients with a diagnosis of sepsis, severe sepsis or septic shock, but all patients, improves patient outcomes and decreases mortality. Improving nurse to nurse or nurse to physician communication decreases time away from implementing interventions. I will use the data I collected from implementing my quality initiative handoff tool to determine if it is efficient enough to be used in practice.

The Application of Data to Problem-Solving

In the modern era, there are few professions that do not to some extent rely on data. Stockbrokers rely on market data to advise clients on financial matters. Meteorologists rely on weather data to forecast weather conditions, while realtors rely on data to advise on the purchase and sale of property. In these and other cases, data not only helps solve problems, but adds to the practitioner’s and the discipline’s body of knowledge.

Of course, the nursing profession also relies heavily on data. The field of nursing informatics aims to make sure nurses have access to the appropriate date to solve healthcare problems, make decisions in the interest of patients, and add to knowledge.

In this Discussion, you will consider a scenario that would benefit from access to data and how such access could facilitate both problem-solving and knowledge formation.

To Prepare:

  • Reflect on the concepts of informatics and knowledge work as presented in the Resources.
  • Consider a hypothetical scenario based on your own healthcare practice or organization that would require or benefit from the access/collection and application of data. Your scenario may involve a patient, staff, or management problem or gap.
