There are times when a nurse must provide care for a client who is agitated and verbally or physically aggressive. The client may be psychotic, but the nurse must, in each case, remain in control of his/her own feelings, and intervene in the most appropriate manner.

After watching a segment regarding a violent client at Bellevue Hospital, answer the questions below. It’s not necessary to critique the staff in the video, just think of how you might respond.

Assessing the Quantitative Analytical Approaches in Health Care Literature

  • For this assessment, you will develop a 3-4 page critique of the quantitative design, methods, and results of a scholarly study.
    The ability to use quantitative approaches to analyze health care data is a vital skill for today’s doctoral prepared professional. You will be expected to have the skills to critically assess the deeper analytical qualities of an article and ultimately comment on its overall validity and practical relevance. This assessment will provide you with an opportunity to demonstrate and hone your ability to analyze and critique the quantitative methods of a research study using an example from the literature.
    For the learner who has finished the data collection process for the doctoral project, analysis of that data offers an exciting (and sometimes challenging) opportunity of discovery. One of the most common statistical techniques for examining the relationship of two variables is correlation analysis. The specific kind of correlational technique depends on the combination of the measurement level (that is, categorical, ordinal, or interval or ratio) of the two data variables being examined. Correlation analysis can tell us the direction and strength of relationships between two variables.
    Overview
    Whether preparing a scholarly document for your doctoral program or simply trying to stay current in your professional field, you must continuously grow in your ability to read research. As an undergraduate, you probably just skimmed over an article’s abstract and introduction, focusing most of your attention on the interpretation of the results at the conclusion. As a doctoral-level professional, your colleagues will expect you to have the skills to critically assess the deeper analytical qualities of an article and ultimately comment on its overall validity and practical relevance.

Hello Marial, Thanks for sharing your response. Please perform an evaluation of your experiences as a professional nurse. Reflect on a patient experience where you encountered a health disparity that was a determinant to the patient’s health. In hindsight, was there an opportunity where you should have acted to help eliminate this but did not? Explain. What would you do differently next time you encounter a similar situation?

William Thompson is a 38-year-old, African American, Catholic who was recently married to Luli Kim. He was a captain in the military during the Iraq war. William is a lawyer specializing in finance law and in jeopardy because of alcohol and PTSD related concerns. He became homeless when he was unable to pay his mortgage. The patient stated, hitting some hard times and needed to move back in with his brother and his brother’s wife. He also stated, they say I have PTSD but that’s another story. William is a marathon runner, plays soccer, enjoys listening to jazz, and is a novice modern art collector.

DSM-5 Criteria

According to the scenario given, the client William Thompson doesn’t have enough signs and symptoms nor enough information to officially diagnose him with PTSD.  According to the DSM 5 criteria requirement, a person being diagnosed with PTSD must have been exposed to actual or threatened death, serious injury, or sexual violence either directly or by experiencing. The traumatic event also needs to be a recurrent event, involuntary, and intrusive.  (DSM-5, 2020). Unfortunately our patient William Thompson does not meet those criteria-based on the information given and information available on patient clinical records.

Client Does Not Meet Criteria 

 As a future nurse practitioner, I will do the ethically right thing and not give my patient any diagnosis if they do not meet the criteria. I believe that labeling the patient based on family history and not on signs and symptoms is wrong. I will conduct a future mental health assessment of the patient to see what the right diagnoses are for the patient and how I can develop an effective treatment plan (Flanagan et al., 2016).

Goals of Treatment

 According to the Psychiatric Association’s practice guidelines for PTSD goals of treatment should include managing signs and symptoms; preventing Comorbid related symptoms such as depression, insomnia, substance abuse, and pain; improve functioning; improve the sense of trust and safety; protect against relapse; and transfer experience into a constructive plan of safety, prevention, and protection (Szafranski et al., 2017). The most important goal of treatment for the patient will include preventing him from committing suicide or hurting those around. The safe of the client and others should always be a priority (Flanagan et al., 2016).

Care plan Options

            As care providers, I will consider using psychotherapy with the client in conjunction with medication therapy that can manage signs and symptoms of depression, insomnia that might be preventing the client from completing his day today activities. Some medication that I will prescribe will include Zoloft to help with the mood and Ambien to help with insomnia, I will also make sure the client enroll in cognitive behavioral therapy (CBT).

Capstone Project Framework

The purpose of the signature assignment is for students to apply the research and EBP concepts they have learned in this course and develop a framework for the initial steps of the student’s capstone project. The assignment allows the student to initiate the steps for planning, researching and developing an evidence-based practice intervention project proposal. This formal paper will include and expand upon work completed thus far in prior assignments.

Essential Components of the Final Project Proposal will include:   

  1. Introduction– Provide an introduction to your topic or project. The introduction gives the reader an accurate, concrete understanding what the project will cover and what can be gained from implementation of this project.
  2. Overview of the Problem – Discuss the problem,  why the problem is worth exploring and the potential contribution of the proposed project to the discipline of nursing.
  3. Project Purpose Statement – Provide a declarative sentence or two which summarizes the specific topic and goals of the project.
  4. Background and Significance – State the importance of the problem and emphasize what is innovative about your proposed project. Discuss the potential impact of your project on your anticipated results to the betterment of health and/or health outcomes.
  5. PICOt formatted Clinical Project Question(s)– Provide the Population, Intervention, Comparison, Expected Outcomes and Timeframe for the proposed project.
  6. Literature Review – Provide the key terms used to guide a search for evidence and discuss at least five (5) summaries of relevant, credible, recent, evidence-based research studies to support the project proposal.
  7. Critical Appraisal of Literature – Discuss the strengths and weaknesses of the evidence, what is known from the evidence and what gaps in evidence were found from the appraisal of evidence-based research studies.

    Emma feels that it is important to provide hands-on learning (e.g., role play) while Isabella feels that a lecture is the most appropriate format for this team. Emma defends her point by saying that her sister has been a teacher for 10 years, and she has proven that hands-on learning increases the engagement and likelihood of remembering the training material.

    Isabella feels that Emma does not account for her 10 years of experience in the medical office and voices her disgust with Emma and her lack of appreciation for her experience. Isabella storms off and leaves Emma to finish the work.
