Vid reflection

Select one of the organizational or behavioral change models which were used successfully for evidence-based practice change in healthcare for many years. Select one model or theory from the readings or lesson this week and:
• List the steps or the components in the change model or theory.
• Does the model or theory contain a component for appraising the evidence?
• Does the model or theory contain a component for networking with the stakeholders during all phases of practice change?
• Does the model contain components for identifying barriers and addressing barriers to implementation?

Your comprehensive health history that was assigned in Module 01 is 10/28/20 due. Complete a comprehensive history, utilizing the form linked below, on either someone over the age of 65 or someone that you know has a lot of medical problems. Write the results in narrative format and include the family history as a genogram (see your text).

Discuss reasons for applying to the Accelerated Bachelors of Science in Nursing program at the XXXXXX and how it fits into personal, educational, and professional goals.

n 500-750 words, develop an evaluation plan to be included in your final evidence-based practice project. Provide the following criteria in the evaluation, making sure it is comprehensive and concise:

1. Describe the rationale for the methods used in collecting the outcome data.
2. Describe the ways in which the outcome measures evaluate the extent to which the project objectives are achieved.
3. Describe how the outcomes will be measured and evaluated based on the evidence. Address validity, reliability, and applicability.
4. Describe strategies to take if outcomes do not provide positive results.
5. Describe implications for practice and future research.

1. Read the information about the case study with the chart materials provided or gather the necessary information from one of your patients.

2.Review the patient assessment information. What other assessments might you do in addition to those provided? What findings would you anticipate with this patient? Develop a care plan (use the template provided) based on the highest priority problem (1 nursing diagnosis, 2 goals, 3 interventions for each goal, 2 rationales for each intervention and evaluation for each goal). Complete an in-depth analysis reflecting your ability to prepare a case study based on principles derived from pharmacology, pathophysiology, psychology, nutrition, and evidence-based nursing practice guidelines. Remember to include an introduction to the patient, pathophysiology, history, nursing assessment (given and any additional), and related treatments.

3.Include selected references for your case study paper/care plan and use APA format.

4. A specific grading rubric and template are attached. The care plan should also be completed.

Instructions for Answer to 2 Question

1- After Each DQ (question), write down references

2- 300 minimum words for every DQ, you can go up to 800 words but answer should be complete.

3- 2-3 Peer Reviewed/ scholarly references for each question

4- References should be within 4 years

5- I am in acute care nurse practitioner program.

6- The response to the DQ is expected to be a minimum of 300 words. A minimum of two peer reviewed/ scholarly  resources are expected. These need to be appropriate for a clinical professional to guide decisions about patient care. If a textbook is used for one of these responses, the other needs to be journal or professional-level website. The references need to be correctly formatted, as do the citations for those references.  “ Question words” don’t count towards 300 minimum count”
