This week’s assignment is STEP 2 – Project Purpose Statement, Background & Significance and PICOt Formatted Clinical Project Question.

The student will answer the items below in a Word doc and submit to the assignment link.

1. Provide a title that conveys or describes the assignment.
2. Project Purpose Statement – Provide a declarative sentence or two which summarizes the specific topic and goals of the project.
3. Background and Significance – State the importance of the problem and emphasize what is innovative about your proposed project. Discuss the potential impact of your project on your anticipated results to the betterment of health and/or health outcomes.
4. PICOt formatted Clinical Project Questions – Provide the Population, Intervention, Comparison, Expected Outcomes and timeframe for the proposed project.
5. References – Cite references using APA 7th ed format.

My PICOT question is 

For elderly patients above 60 years with pressure ulcers (P) will the negative pressure wound therapy (I) as opposed to standard moist wound therapy (C) improve the healing of the pressure ulcer (O) during their two-week stay at the hospital (T).

Individuals and a Family as a Client 

Mila Jefferies is a recently widowed 36-year-old mother of two children and the daughter of two aging parents in the southeastern United States. She and her children have recently relocated from an urban neighborhood to a rural town to care for her parents, Robert and Susan. The move involved a job change for Elizabeth, a change in schools for the children, and an increased distance from the family of the children’s deceased father. Mila’s older child is a 5-year-old daughter, recently diagnosed with autism spectrum disorder and dyslexia. The younger of the two children is a 3-year-old boy with asthma that has been difficult to control since the move. Robert is a 72-year-old Methodist minister who recently suffered a stroke, leaving him with diminished motor function on his left side and difficulty swallowing. Susan is 68 years old and suffers from fibromyalgia, limiting her ability to assist with the daily care of her husband. She has experienced an increase in generalized pain, difficulty sleeping, and worsening fatigue since her husband’s stroke. 

Use the Neuman systems model as a conceptual framework to respond to the following: 

• Describe the Jefferies family as a client I system using each of the five variables. 

• What actual and potential stressors threaten the family? Which stressors are positive, and which are negative? Separate the actual and potential stressors that threaten the individual members of the family. Which of the stressors are positive, and which are negative? 

• What additional nursing assessment data are needed considering Robert’s medical diagnoses? What additional data would be helpful for Susan’s medical diagnoses? What about each of the children? 

• What levels of prevention intervention(s) are appropriate for the Jefferies family? Propose potential prevention intervention(s) for each member of the family. 

• Identify your nursing priorities if you were providing care to this family. 

Not all EBP projects result in statistically significant results. Define clinical significance, and explain the difference between clinical and statistical significance. How can you use clinical significance to support positive outcomes in your project? (Fall prevention strategies in hospitals.)

4. Identify a clinical topic and related nursing practice issue you think needs to be changed. 
5. Locate a systematic review on your topic from the CCN Library databases. Be sure this involves nursing actions.
6. Work through each step of the ACE Star Model as outlined on the assignment form (Star Points 1-5: Discovery, Summary, Translation, Implementation, and Evaluation). Respond to the instructions provided on the form.
7. Follow the activities and thinking of Nurse Daniel in Weeks 1-6 in the ‘Illustration’ part of each lesson. He will be working through a clinical topic and nursing practice issue to demonstrate a change (ACE Star Model and systematic review).
8. Work on a portion of the process each week, as the illustration unfolds.

As a nurse informaticist, you may be at the forefront of managing patient data, institutional data, and other record-keeping that has reporting requirements. The reporting of accurate and truthful information is critical to policy compliance, and may affect funding, safety ratings, or other crucial measures. In this Discussion, you analyze one such example where informatics and ethics overlap. 

For this Discussion, consider the following scenario:

Imagine that you are a nurse informaticist in an ER department. The manager informs you that the duration from when a patient arrives in the ER reporting cardiac-related symptoms to being seen in the cardiac catheterization lab is above the national standard. She asks if there is a way to “adjust” the report to make the time look better before the next review.

• Review the resources and reflect on the actions you would take and the decision you would make to meet standards in an ethical manner as it relates to the scenario provided. 
• Think about what your response to the manager might be.
