Assignment

Patient Preferences and Decision Making

Changes in culture and technology have resulted in patient populations that are often well informed and educated, even before consulting or considering a healthcare need delivered by a health professional. Fueled by this, health professionals are increasingly involving patients in treatment decisions. However, this often comes with challenges, as illnesses and treatments can become complex.

What has your experience been with patient involvement in treatment or healthcare decisions?

In this Discussion, you will share your experiences and consider the impact of patient involvement (or lack of involvement). You will also consider the use of a patient decision aid to inform best practices for patient care and healthcare decision making.

To Prepare:

  • Review the Resources and reflect on a time when you experienced a patient being brought into (or not being brought into) a decision regarding their treatment plan.
  • Review the Ottawa Hospital Research Institute’s Decision Aids Inventory at https://decisionaid.ohri.ca/.
    • Choose “For Specific Conditions,” then Browse an alphabetical listing of decision aids by health topic.

NOTE: To ensure compliance with HIPAA rules, please DO NOT use the patient’s real name or any information that might identify the patient or organization/practice.

By Day 3 of Week 8

Post a brief description of the situation you experienced and explain how incorporating or not incorporating patient preferences and values impacted the outcome of their treatment plan. Be specific and provide examples. Then, explain how including patient preferences and values might impact the trajectory of the situation and how these were reflected in the treatment plan. Finally, explain the value of the patient decision aid you selected and how it might contribute to effective decision making, both in general and in the experience you described. Describe how you might use this decision aid inventory in your professional practice or personal life.

Week 4: Assignment – Application of Concepts from Caring Science

Web Page

Task: View this topic

Points: 300 | Due Date: Week 4, Day 7 | CLO: 3, 4 | Grade Category: Assignments

Assignment Prompt

Summary: A theory can be used to guide practice.  This assignment is an exercise in supporting a clinical practice with theory and evidence.

Directions: Identify an outcome of nursing practice in your area of practice that can be improved.  For example, if you work in home health, you may identify that throw rug use by fall risk patients is too prevalent.  You may be able to use the problem that inspired the theory concepts that you developed in week two. Identify the concept in Watson’s Theory of Caring that could represent or include the outcome.  In our example, the outcome would be the changes in self through the change in the patient’s floor covering practice. Identify a practice that can be changed or implemented that may influence the outcome. Identify the concept in Watson’s Theory of Caring that includes the practice.  In our example, the practice could be to improve the understanding of fall hazards through the engagement in a teaching-learning experience, one of Watson’s Caritas Processes. Identify exactly how these two concepts will be measured with their operational definitions. Develop a proposition between the two. Present your outcome in an APA formatted paper  meeting the University’s standards for a written assignment.

Expectations

· Due: Saturday, 11:59 pm PT

· Length: 5 to 7 pages including title and reference pages

· References: 3 to 10.  There should be enough to support the links between the concepts of the problem and the concepts of Watson’s Theory of Caring.

Discussion Prompt

Find a concept within Watson’s Theory of Caring and a similar concept within any other nursing theory.  Compare the two concepts for similarities and differences. Illustrate how a person reading about a theory can be influenced in their understanding of a theory if they have a different definition of a concept than the author.

write a 2 page paper addressing the sections below of the research proposal.

Methodology

  • Extraneous Variables (and plan for how controlled).

Local Practice Problem Exploration: Cancer

 Local Practice Problem Exploration: Cancer.

As we have chosen the same practice problem, to explore, I am curious how your facility (north shore medical center, Miami, Florida) has planned to (or currently has implemented) becoming an Age-Friendly institution?  Many of the interventions you described echo the “4 M” is of the Age-Friendly Institution: Mentation, Mobility, Medications, and What Matters Most.  How have you seen institutions adapting to this framework in anticipation of the needs of cancer patients and survivors?

– How cancer impacts nurses, healthcare organizations, nursing care, and general health care quality?

– what is the real impact on your community? What is the cost to your local community (Miami, Florida)?

Just as you sought out stakeholders when considering practice problems at the national level, an ongoing dialogue with stakeholders at your local level is equally important. Get to know your formal and informal leaders. Networking with stakeholders sets into motion a collaborative relationship that is needed to focus on a practice problem with the intention of translating the best available evidence.

 – Key stakeholders affected by the problem and ones involved in the resolution. In your current practice, what contributes to the alarming rates across the known practice problems? During your journey this week, you’ll also examine the role of stakeholders in change and the role of translation science in influencing patient outcomes across known practice problems. During your readings, you probably noted that there are challenges and barriers to changing practice.

Personal Theoretical Framework For Advanced Practice Nursing

(I need 2 different copies of this Assignment)

Discuss a personal nursing philosophy.  Apply what you have learned about nursing theory in this course.  Identify a nursing philosophy that best matches your personal philosophy.  Discuss a nursing framework or theory that fits that philosophy including how it fits your personal philosophy.   Identify a possible situation in which that framework or theory would be a poor fit and discuss why it is a poor fit for that situation.  While it is an important skill to be able to match a theory with a situation, it is also critical to understand when a theory or framework does not fit a situation.

Pharmacology week 1 discussion

For these questions, please read the following case study and then respond to the questions noted below.

Ms. BD is a 33-year-old G2P1 African-American female who presents to your clinic today complaining of unusual fatigue, nausea, and vomiting for the last five days. She has a medical history of chronic hypertension (HTN) that was diagnosed shortly after her first pregnancy two years ago and GERD. MS. BD’s blood pressure is controlled on Lisinopril-Hydrochlorothiazide 20/12.5mg by mouth twice a day, and GERD controlled on Bismuth Subsalicylate 262mg by mouth every 6 hours as needed. During the interview, you learn that she is single, sexually active, has one partner and that her menses is ten days late. She performed a home pregnancy the three days after missing her menstrual cycle, and the results were inconclusive. She states she feels terrible and needs relief. She has no other medical problems, symptoms, or concerns.

Assessment: Physical examination is unremarkable. BP128/68, HR is 74, Urine human chorionic gonadotropin (HCG) positive, beta HCG sent, potassium 4.2, blood
urea nitrogen (BUN) 14, creatinine is 0.6, Alanine aminotransferase (ALT) 29, White blood cells (WBCs) 6.5, hemoglobin (Hgb) 12.8, hematocrit (Hct) 39, and platelets 330,000.

  1. List the additional questions you would need to ask this patient. Explain.
  2. What is the safety profile of Lisinopril-hydrochlorothiazide and bismuth subsalicylate in pregnant women? What are the possible complications to the pregnant woman and her fetus?
  3. What is the importance of assessing laboratory values when prescribing medications? How might the laboratory values, in this case, impact your treatment plan?
  4. Would you make any changes to Ms. BD’s blood pressure and GERD medications? Explain. If yes, what would you prescribe? Discuss the medications safety in pregnancy, mechanism of action, route, the half-life; how it is metabolized in and eliminated from the body; and contraindications and black box warnings.
  5. How does ethnopharmacology apply to this patient if she were NOT pregnant? Explain.
  6. What health maintenance or preventive education do you provide in this client case based on your choice of medications/treatment?
  7. Would you treat this patient or refer her? Explain. If you refer, where would you refer this patient?
