  • Preparation As the senior care coordinator in your organization, your manager has asked you to examine and report on how care coordination can generate cost savings, improve outcomes, enhance the collection of evidence-based data, and improve health care quality for the community. She would like you to compile cost savings data in a well-organized spreadsheet and present your key findings in an executive summary. Sgt. Eddie Johns leaned back against the chair in the outpatient orthopedic clinic. His head was killing him! He wasn’t sure which was worse, the “morning after” headache or not being able to sleep at night. At least when he had a few beers under his belt so he could catch a few hours of sleep. It had been like this since he was air evacuated back stateside from Afghanistan after the roadside bomb went off. He was thankful that he had only broken his leg in a couple of places and gotten a bad bump on the head. They called that traumatic brain injury but he didn’t know what that was and really didn’t believe them anyway. He was still thinking just fine. His friend Joe wasn’t so lucky! How was Joe going to learn to walk on those artificial legs? He was still in the hospital in Washington, DC. That was pretty far from his home. Eddie wished he could visit Joe. Knowledge Gaps in Applying Evidence to Practice

 The DNP scholar, who is seeking to learn more about the different types of inquiry, Research, Quality Improvement, and Evidence-based practice, will bridge the gap between knowledge, research, application, and translation of evidence to nursing practice. There will be interrelationships between nursing practice, education, theory, and research. A systematic review is an important tool for the DNP scholar who is seeking evidence (Polit & Beck, 2017). Dang & Dearholt inform us that systematic reviews utilize meta-synthesis and meta-analysis to analyze the results of several studies. Therefore, these two most reliable methods and the highest standards in evidence-based care could lead the DNP scholar; further determine the effects of nursing interventions, prevention, treatment, and rehabilitation. Case-controlled studies, case series, and case reports make the DNP do in depth research of individual units, which gives insight into the specific intervention. Cohort studies considered observational studies where the DNP scholar can be able to answer questions on the disease etiology, diagnosis, and prognosis except interventions. DNP scholar uses random control trials to measure the different interventions therefore make the best clinical decision on the outcome of each intervention. Critically appraised individual articles are less reliable however, it helps to assess the outcomes for evidence doing an individual study. Critically appraised topics help to understand the strength of evidence provided and creates an understanding on how to evaluate and synthesize multiple research studies. The least reliable evidence yet helpful for the DNP scholar is the ideas, opinions anecdotes, and editorials. It is crucial as a DNP scholar to understand the pyramid of evidence (all seven levels) and to identify the levels needed for their study.

A serotonin reuptake inhibitor, such as Fluoxetine, may be prescribed for this patient because she has a disturbed sleep pattern, fatigue, and difficulty concentrating. These three symptoms could correlate with an altered mood. Prozac works by blocking the reabsorption of the serotonin neurotransmitter and causing an increase of the amount of serotonin in the brain. It is believed to increase the client’s mood, improve sleeping patterns, and enhance the thought process. This medication is commonly used to treat major depressive disorders (Henry et al., 2016).

Some important teachings to give the client when administering the first dose are to avoid caffeine, monitor daily weights because weight loss is a common adverse effect in the early stages of therapy, monitor for the signs and symptoms of serotonin syndrome such as increased body temperature, fast heart rate, frequent loose stools, sweating, tremors, and/or anxiety, watch for bleeding or bruising if the medication is given with a blood thinner, and advise the client that fatigue or dizziness can occur (Henry et al., 2016). Serotonin syndrome can occur when there’s an excessive amount of serotonin in the body and can cause changes in mental status and heart function. If the patient observes any of the symptoms listed above, she should notify the physician and nurse immediately

Some changes I expect to see with the client after 2 months of treatment are increased appetite, reduced fatigue, return to a normal sleep pattern, improved concentration and thought process, reduced thoughts of guilt, improved mood, and the patient appears to be content with her appearance, mood, and thoughts. If the client is not displaying the expected outcomes, the physician may prescribe an SNRI such as Venlafaxine, an atypical antidepressant such as Bupropion, or a monoamine oxidase inhibitor such as Phenelzine.
