In healthcare, the advances in technology are vast and provide the foundation for the future of healthcare information delivery systems. The modernization of healthcare has changed the evolution of documentation and education to be available whenever it is needed. Technology improves access to evidence-based practice; it also supports access to the information within the clinical setting, which improves care and the planning of care. Technology helps facilitate communication within the healthcare team; it also helps maintain standardization of practice pattern. It accelerates surveillance for error detection and may also assist in preventing errors.   

The electronic medical record (EMR) can be a beneficial tool in implementing new processes like adopting formal wound care documentation. In this capacity quality, process improvement tools can also be implemented to track pressure injuries in real-time and identify factors that contributed to it more efficiently. When a wound is documented, it can alert the wound care team, and then they can assess it and assist with documentation while developing a course of care action. This can also be helpful when patients are admitted to alerting the RN to chart skin assessments within 24 hours using the four eyes wound care documentation tool. The Braden scale is also a tool that nurses chart daily, and if their Braden score increases, it notifies the wound care team to assist in a plan to prevent pressure injuries before they happen. I plan to use this tool after implementation to track injuries and evaluate areas we need improvement on when it comes to preventing pressure injuries. The most significant barrier I foresee is the lack of documentation by staff or missing opportunities for improvement while the change is in process. Another barrier is that it can take time to build these processes into our charting system; we use Cerner, which has pre-set charting for wounds, which would have to be altered by our IT staff.

I have been working in the medical field for almost 20 years. Since then I have been trying my best to work upon and improve one of my weakness, and that is my shy nature. I understood that my weakness can be an obstacle in making relationships among my colleagues and clients. I find it very hard to initiate talks with colleagues, patients or relatives other than communication pertaining to work or patient information. I have sought help and have definitely improved than before, but I think I still have a long way to go.

Workplace conflict is inevitable when employees of various backgrounds and different work styles are brought together for a shared business purpose. I once had a conflict with a patient when I tried to comfort him by expressing my views of faith healing, which clearly contrasted with his. The situation had escalated for a while, but me and manager intervened by explaining and understanding his point of view and perspective towards the situation.

I am a foreign medical graduate. I practiced critical care medicine in India for five years before migrating to the USA. As I was unable to obtain residency in spite of all efforts, I chose to do nursing and continue to pursue my passion of serving and healing the needy people. I also love learning and gaining deeper education and knowledge pertaining to medicine, hence I decided to advanced nurse practice studies. I am surely looking forward to do my doctorate in nursing in the future.

For decades now, evidence based practice is believed to improve quality patient care in health care where there are practiced. Maximum efforts has been put to eliminate traditional way of doing things and embrace new methodologies (Saunders, Vehviläinen-Julkunen & Stevens, 2016). Concerns have been raised towards improving knowledge and skills in respect to evidence for efficient clinical outcome. Lack of EBP knowledge has bring a lot of challenges in our organization thus hindering its implemented. This has influenced integration of sources of evidence in clinical decision making where clinical expertise, clinical context, patients outcomes and values as well as best research are included in the process of decision-making.

There is increased demand to create culture that promotes exploration of critical inquiry and availability of evidence as an important aspect to be incorporated in our daily nursing practice in the organization. Clinical inquiry in nursing follows five EBP processes including ask, gather, appraise, act, evaluate and disseminate. Learning these steps helps to support the capacity of nurses. According to Mick (2017), the first process to clinical inquiry is developing a clinical question, followed by gathering and evaluating evidence obtained. After finding a satisfactory evidence, actions should be taken for its implementation and dissemination and examining outcome.

As discussed in week one, the significance of evidence-based practice (EBP) in providing safe and quality health care cannot be relegated. In fact, EBP has been endorsed in research and practice as a method that obtains the highest level of care for patients, reduces the cost of health care, reduces medical errors and injuries, and encourages individual and professional advancement for nurses and other health professionals because it encourages research and educational progression. Accordingly, Black et al (2015) concluded that providing research training opportunities to nurses is a worthwhile strategy for health care facilities seeking to promote EBP for the purpose of empowering nurses and consistently ensuring safe and quality health care.

Studies have shown that several issues or factors are implicated in achieving evidence-based nursing practice (e. g. Mahmoud and Abdelrasol, 2019; Solomons and Spross, 2011). Some of these factors include inadequate time to read literature, intense workload, shortage of staff qualified in EBP, and shortage of nursing faculties to inculcate the knowledge of EBP in nurses. Despite the presence of a couple of qualified RNs, the issue of intense workload and inadequate time to read literature is the main issue for my organization in addressing the issue of evidence-based nursing practice. To address this issue however, management must increase the number of nursing staff so that nurses will have more time for investigative inquiries to improve practice. Furthermore, encouraging a culture of research amongst nurses, by keeping nursing journals, articles, and periodicals can be promising for resolving EBP issues in nursing practice.
